1997 FAA Letter: Installation of Continental O200 Engine De-rated to 90 HP in a J3 Cub

This is an FAA Letter regarding the installation of an O-200 in a J3 Cub. It's a copy of a copy of a copy and is barely legible. I've also attached an interpreted version of it but there are some words that I cannot make out and some of what I have interpreted may be incorrect.
Does anyone have a better version of this or an equivalent letter that can be shared? I'd also be receptive to any assistance in interpreting any of the remaining XXX-# words in the interpreted version or correcting any interpretation errors..
